What does each treatment aim to do?

Root canal treatment aims to prevent or heal inflammation at the root tip by cleaning out the inflamed or infected pulp inside the tooth. The tooth stays in the mouth; its root and the surrounding bone and gum are preserved in their natural state. An implant consists of a titanium or zirconia screw placed in the jawbone in place of the extracted tooth, plus the crown/prosthesis on top of it. An implant replaces a lost tooth; it does not save the tooth itself.

So the real question is not “which is better?” but “can this tooth be saved predictably?”

Can the tooth be saved? The criteria your dentist looks at

• Remaining tooth structure: After root canal treatment, will enough sound tooth structure remain to support a filling or crown? Having some sound wall above the gum line (a ferrule) is important for longevity.
• Root integrity: A vertical root fracture usually means the tooth will be lost. When a crack is suspected, CT and clinical tests are assessed together.
• Gum and bone support: If much of the bone around the root has been lost to advanced gum disease, a tooth whose canals were successfully treated may still be loose.
• Canal anatomy and previous treatment: Severely curved or calcified canals, a broken instrument or a failed old root filling make treatment harder but often not impossible. The American Association of Endodontists (AAE) case difficulty assessment form classifies such situations as “high difficulty” and recommends specialist assessment.
• The tooth’s strategic importance: A tooth that will serve as a bridge abutment is assessed differently from a tooth standing on its own.

When does root canal treatment come first?

For teeth whose pulp is inflamed or dead but whose root is sound and restorable, root canal treatment is usually the less surgical, quicker and more tissue-preserving option. According to the studies compiled in the endodontics manual we refer to, root canal treatment performed under proper conditions has a high success rate in teeth without a lesion at the root tip; when an established lesion is present, the rate falls somewhat but remains high. Even when an old root canal has failed, retreatment or root-end surgery (apicoectomy) often offers a chance to keep the tooth.

When might extraction and an implant make more sense?

• A vertical root fracture or a crack running the full length of the root
• Decay or a fracture extending below the gum line that is too large to restore
• A tooth with advanced bone loss and marked mobility
• Infection that does not heal despite retreatment and surgery
• Situations where, in the prosthetic plan, the tooth cannot be a reliable long-term support

Root canal treatment vs. implant: a comparison

Systematic reviews report that, in suitable cases, restored root-canal-treated teeth and single-tooth implants have similar long-term survival rates. In other words, the two treatments are not in a “race”; with the right indication, both are successful. The difference comes from the features of the case.

How is the decision made?

At the examination, the history of the pain is taken, tests such as cold testing, percussion and palpation are performed, and a periapical X-ray is taken. If a crack, root fracture or complex anatomy is suspected, a three-dimensional cone beam CT (CBCT) may be requested. Is it a problem to leave the gap empty for a long time after extraction?

When a tooth is extracted, the surrounding bone gradually loses both height and width. Neighbouring teeth can tip into the gap, the opposing tooth can over-erupt towards it, and the bite can be disturbed. So when extraction is decided, the timing of the implant should also be planned from the outset: in suitable cases the implant can be placed in the same session as the extraction; if there is infection or insufficient bone, healing or a bone graft may be needed first. On the other hand, when a root canal is chosen, the permanent restoration should not be delayed either; a root-canal-treated tooth left open or waiting a long time with a temporary filling can fracture or become reinfected. On either path, completing the plan is at least as important as starting it — something international patients should keep in mind when planning their trip to Istanbul.

Which option may suit whom?

In young patients and in teeth with good bone support, keeping the natural tooth is particularly valuable, because even with an implant, the surrounding tissues can change over the years. Uncontrolled diabetes, heavy smoking or certain medications can affect both healing after a root canal and the implant’s integration with bone; these conditions are assessed separately in the treatment plan. Whichever path you choose, regular check-ups and good oral hygiene are the foundation of longevity.

Finally, it is important not to rush the decision. If there is severe pain or swelling, the complaint is first relieved with emergency care (opening and cleaning the canal, and drainage if necessary); the choice of definitive treatment is made after a calm assessment in which all options and their possible outcomes are discussed. When you are unsure, getting a second opinion is your natural right.
