When is root canal treatment needed?
- Toothache that starts with hot or cold, lasts a long time and gets worse at night,
- Deep decay or a fracture reaching the pulp,
- Discolouration of the tooth, swelling of the gum or a draining fistula (abscess),
- Inflammation at the root tip on the X-ray (periapical lesion),
- When required before a prosthesis or zirconia crown.
How is root canal treatment performed?
- Assessment of the canal anatomy with X-rays and, if needed, 3D CT (CBCT),
- Local anaesthesia and isolation (rubber dam),
- Removal of decay and access to the pulp chamber,
- Shaping of the root canals with rotary instruments and disinfection with thorough irrigation,
- Sealed filling of the canals,
- Restoration of the tooth with a permanent filling or crown.
Root canal retreatment
If pain, swelling or inflammation at the root tip persists in a tooth that has already had root canal treatment, the old filling material is removed and the canals are cleaned and filled again. Where this is not suitable, root-end surgery (apical resection) may be considered — see oral and maxillofacial surgery.
Endodontics, or extraction and an implant?
If it is possible to save your own tooth, root canal treatment is usually the first choice. For root fractures, advanced bone loss or teeth too damaged to be restored, extraction and an implant are considered. A periodontal assessment of the bone and gums supporting the tooth is carried out; in endo-perio lesions both areas are planned together.

Is pain normal after root canal treatment?
Mild sensitivity for 2–3 days is normal. If you have severe pain or swelling, contact the clinic.
How long does a root-canal-treated tooth last?
Correctly treated and well-restored root-canal-treated teeth can remain in the mouth for many years.
Can root canal treatment be done during pregnancy?
In urgent cases it can be done, preferably in the second trimester and after the dentist’s assessment.
